Dear Parents/Caregivers
An important part of your child’s Health and Physical Education in Term 4 is participation in a Surf Education / Surf Skills Program for year 5 and 6 students. The program will be delivered by Noosa Heads Surf Life Saving Club on the following Wednesdays: 11/10, 18/10, 25/10, 01/11. The program is fully supervised by qualified lifeguards and costs $80.00 per student (including bus fare).
Year 6S, 6M, 6H: 9:00am – 12:30pm (on the beach 9:30am – 11:30am)
Year 5C, 5R, 5B: 11:15am – 2:30pm (on the beach 12:00pm – 2:00pm)
Each program session runs for 2 hours and includes:
- Surf/Beach Awareness: - Water conditions, tides, currents and other hazards, sun care, beach regulations and signs/flags.
- Surf Skills and Fitness: - Wading/porpoising, surf swimming, body surfing, survival flotation.
- Board Paddling Skills: - Board paddling, negotiating breaks, board rescues, boogie boarding.
- Resuscitation and Introduction to First Aid: - Common beach first aid cases.
Children will travel to and from the venue by bus. Please note that students will need to take their morning tea and/or lunch with them as there will be time to eat after each session.
Payment, along with this online curriculum activity consent form are to be completed by Tuesday 10thOctober – thank you. It is essential that this $80.00 fee be paid prior to this due date as the school must adhere to a ‘no pay, no go’ policy. There are various payment options available including: BPoint, eftpos and cash. Costings are worked out on full participation; however, refunds will be made available upon request for three or more missed sessions.
We thank you for your co-operation in ensuring payments (or ‘Payment Contracts’) have been made for your child/children. The school can work with you on setting up a “Payment Contract” if you require - individual circumstances can be taken into account. If you are in this situation you will need to notify the office before the DUE DATE of a payment so we can determine if a “Payment Contract” can be set up based on your circumstances.
Privacy notice
The Department of Education is collecting the personal information requested in this form in order to:
- obtain lawful consent for your child to participate in the activity;
- help coordinate the activity;
- respond to any injury or medical condition that may arise during, or as a result of the activity; and
- update school records where necessary.
The information will only be accessed by authorised school staff and will be dealt with in accordance with the confidentiality requirements of, as applicable, s.426 of the Education (General Provisions) Act 2006 (Qld), the Information Privacy Act 2009 (Qld), and/or the Privacy Act 1988 (Cwlth).
The information will not be disclosed to any other person or agency unless the disclosure is authorised or required by law, or you have given the department permission for the information to be disclosed.
Activity Risks & Insurance
Please note that the Department of Education and Training does not have personal accident insurance cover for students. If your child is injured as a result of an accident or incident while participating in the activity, all costs associated with the injury, including medical costs are the responsibility of the parent/carer. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may be also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.
